Kaiser will allow an
eligible employee who
originally declined coverage to enroll outside normal open enrollment rules
provided the situation considered is a

dependent must enroll). Other enrollment reasons are as follows: Part-Time
to Full-Time Employment Status, Moved into Service Area, Rehired within 6
months, Return from Layoff/Leave of Absence, Return from Military Duty.
Kaiser also has a 60 day retro rule. If an
employee or dependent enrolls within 60 days pass the eligible effective
date, the member will be enrolled retro to the date he/she was eligible. For
example, the employee is eligible effective 10/1/03. Kaiser receives
application on 11/5/03. The member's effective date will be 10/1/03.
