• $0 or low doctor visit copays, depending on where you live

• $0 outpatient surgery copays, (Los Angeles, Orange and San Bernardino counties only)

• Basic health club membership (Los Angeles, Orange and San Bernardino counties only)

• Coverage through the Medicare Part D coverage gap with $0 or low copays for formulary generic drugs (Los Angeles, Orange and San Bernardino counties only)

• Partial coverage for preferred brand drugs in (Los Angeles and Orange counties)

• Preventive dental coverage (Los Angeles, Orange and San Bernardino counties only)
